5 Things That Actually Help

1. Physiotherapy — the only intervention that works across all 3 stages

Physiotherapy is the single best-evidenced intervention for adhesive capsulitis, and the only one with strong evidence across all three stages. But the type matters enormously — generic shoulder exercises designed for rotator cuff rehab or general shoulder conditioning are not the right approach for adhesive capsulitis. You need a physiotherapist who specifically treats frozen shoulder and who will progress your treatment based on your current stage. In the freezing stage, that means gentle range-of-motion work that doesn't provoke acute inflammation. In the frozen stage, manual therapy and progressive stretching. In the thawing stage, active mobilisation and strengthening. Early intervention shortens total recovery time significantly — the difference between 8 months and 18 months is often whether physiotherapy began in the first three months.

4. HRT — worth a specific conversation about the shoulder

There is growing evidence that estrogen therapy reduces the inflammatory response that drives adhesive capsulitis — which is biologically coherent given that estrogen receptor activity in the joint capsule is the root mechanism. Some evidence suggests women on HRT have lower incidence of adhesive capsulitis and faster recovery when it does occur.   • Wait and hope it resolves on its own. It will eventually — but the natural untreated timeline is 1–3 years, and full restoration of range of motion is not guaranteed without intervention.
